
BayWoods has an immediate opening for a Wellness Nurse. The Wellness Nurse is responsible for implementing and providing a Health and Wellness program to the Independent Living community. Responsible for assisting with the implementation of the Employee Health program.
Duties:
· Respond to any emergency resident call within the community.
· Assess and provide necessary medical services to independent residents
· Review all physician reports on Independent Living candidates preparing for admission to the Health Care Center
· Keeps clinic hours for medical appointments with health care providers.
· Organizes and completes weekly lab orders from IL residents’ physician.
· Notify IL residents’ and their physicians of lab results.
· Document all services rendered each day and complete accurate billing logs as required
· Documents all communications with physicians, family members, pharmacists etc.
· Develop health promotion and education programs as well as special events for independent living resident population.
· Coordinate with the Director of Health and Wellness/Director of Resident Services to develop special events, health fairs, and speaker
· Programs for independent living resident population.
· Maintains accurate and complete medical records.
· Assess the residents in the Independent Living facility for possible transfers to the Assisted Living
· Participates in the orientation of new hires.
· May assist Nurse Navigator with Employee Health program.
· Provides nursing care according to physician’s orders and conformance with recognized nursing practice, established standards, and administrative policies.
· Ensures preventative, therapeutic, and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
· Has the skill to assess resident and notify attending physician and responsible party in the event of a change in the resident’s condition.
· Assesses and monitors complex resident health status and determines the appropriate intervention.
· Develops and monitors the ongoing plan of care with resident and initiates corrective action as necessary
· Monitors resident environment for safety and sanitation.
· Orients residents and staff to emergency procedures and responds to all emergency situations.
· Completes and signs incidents reports and provides follow-up as required.
· Assists in direct resident care, as necessary.
· Maintains accurate documentation of medication administered and care rendered.
