Social Services Assistant

Location: Venza Care at Rocket City Rehabilitation and Healthcare Center

Shifts:

1. Days – Full-Time

Key Responsibilities:

1. Assist with resident admission, discharge, and transfer planning
2. Support the Social Services Director with assessments and documentation
3. Maintain and update social service records in accordance with facility policy
4. Serve as a liaison between residents, families, and facility staff to resolve concerns
5. Help coordinate referrals to community agencies and services
6. Participate in interdisciplinary care planning and resident care meetings
7. Provide emotional support and advocacy for residents and their families
8. Ensure residents’ rights and confidentiality are always respected

Qualifications:

1. Prior experience in Social Work
2. Prior experience in a long-term care or healthcare setting preferred
3. Strong organizational, communication, and interpersonal skills
4. Compassionate and patient-focused approach
5. Ability to work well in a team and manage multiple responsibilities
