Role Overview:

As a Social Services Coordinator at Five Oaks Rehabilitation and Care Center, you will play a crucial role in enhancing the quality of life for our patients. You’ll assess patients' needs, develop care plans, and collaborate with medical staff to provide comprehensive care.

Key Responsibilities:

1. Conduct patient assessments to determine their social service needs.
2. Develop and implement personalized care plans in coordination with an interdisciplinary team.
3. Provide counseling and emotional support to patients and their families.
4. Advocate for patient needs within the facility and in the broader community.
5. Maintain detailed and confidential patient records.
6. Oversee discharge planning and coordinate with community resources to ensure continuity of care.

Qualifications:

1. Bachelor's Degree in Human Services, Psychology or equivalent
2. 2 or more years exp in skilled nursing facility
3. Experience in long-term care health settings, particularly in social services.
4. Strong understanding of case management and care coordination.
5. Exceptional communication and interpersonal skills.
6. Ability to work compassionately with a diverse patient population.
